49 local businesses receive critical violations in May 2024 food inspection report

The Monroe County Health Department released its May 2024 food inspection report, with 49 locations receiving critical violations.  

According to Monroe County’s website, critical violations are defined as incidents likely to directly contribute to a foodborne disease.  

AFC Sushi at Kroger J330: 1825 N. Kinser Pike 

One violation was due to refrigerated food being held for over 24 hours without a date marking 

Arepa Burger: Food Truck Friday 

One violation was due to debris and buildup on the hood and hood screens.  

Avers Pizza (North): 1837 N. Kinser Pike 

One violation was due to debris buildup on pop nozzles.  

Big Woods Bloomington: 116 N. Grant St.  

One violation was due to debris buildup in the ice machine.  

Bloomingfoods Market & Deli: 3220 E. Third St.  

One violation was due to sliced chicken, tofu and marinated mushrooms stored at 54°F - 48°F and mashed potatoes, beef stew and boiled rice stored at 48°F.  

Bloomington Superior Fuel doing business as Third Street Express: 4001 W. Third St.  

One violation was due to five dented cans on a shelf.  

Boba One: 2894 E. Third St., Kiosk 213 

Two violations were due to: 

Burger Theory at the Holiday Inn: 1710 N. Kinser Pike 

Two violations were due to:  

  • A live rodent found near the doors leading into the kitchen. 
  • Debris buildup on the ice machine. 

Cascades Inn: 2610 N. Walnut St.  

One violation was due to debris buildup on the ice machine.  

Cloverleaf: 325 E. Winslow Road  

One violation was due to raw turkey breast being stored in the reach-in cooler above milk and ready-to-eat sausage gravy. 

Comfort Inn: 1700 N. Kinser Pike 

One violation was due to food being held for more than 24 hours with no date marking.  

Community Kitchen Food For All: 1515 S. Rogers St. 

One violation was due to the hand washing sink being used to refill water reservoirs.  

Dairy Queen: 2423 S. Walnut St. 

One violation was due to unlabeled Temperature Control for Safety food in the walk-in.  

Dollar General #1815: 3618 W. 3rd St. 

One violation was due to multiple cans having dents on the seams.  

Dunkin Donuts: 300 S. College Mall Road  

One violation was due to the sanitizer dispenser in the Baskin Robbins area and all sanitizer buckets measuring zero PPM.  

Fat Dan’s Deli: 221 E. Kirkwood Ave., Ste. 1  

One violation was due to a buildup in pop nozzles behind the bar and in the ice machine in the back.  

Fazoli’s #1627: 315 College Mall Road 

One violation was due to debris on pop nozzles.  

Feta Kitchen: 600 E. Hillside Drive  

One violation was due to a meat mixture in the reach-in cooler with no labels and being stored at 46°F. According to the CDC, deli meats should be stored at a temperature of 41°F or below. 

Food Mart (Ellettsville): 7398 W. State Road 46 

One violation was due to pest droppings found near the mop sink and underneath the soda dispenser.  

Great Taste Buffet: 3616 W. Third St. 

Two violations were due to: 

  • Debris buildup around the faucets on the handwashing sinks. 
  • Ready-to-eat food without date markings in the walk-in cooler.  

Heri #14 doing business as On The Way #14: 4723 W. State Road 46 

One violation was due to refrigerated, ready-to-eat and potentially hazardous foods held for more than 24 hours without date markings.  

Holiday Inn Express: 117 S. Franklin Rd. 

Two violations were due to: 

  • A bucket inside of the hand sink in the food prep area. 
  • No date markings on food held for more than 24 hours.  

Hoosier Hanks East: 1285 S. College Mall Road 

Had three violations due to: 

  • Pulled chicken measured at 122°F and cold pulled chicken measured at 46°F. 
  • Food debris buildup in the microwave. 
  • Dishwasher sanitizer measured at zero PPM.  

Jersey Mike’s: 2618 E. 10th St. 

One violation was due to buildup in pop nozzles and ice nozzles.  

Jimmy John’s: 1827 E. 10th St.  

One violation was due to buildup in the ice machine.  

Juanita’s Restaurant: 620 W. Kirkwood Ave.  

One violation was due to the dishwashing machine not dispensing the proper sanitizer.  

KFC H740028 (West): 3700 W. Third St.  

One violation was due to hoses in three bay sinks with no air gap.  

Kona Ice 1: Food Truck Friday 

One violation was due to having no chemical sanitizer available. 

La Charreada: 1720 N. Walnut St.  

One violation was due to refrigerated and ready-to-eat food being held longer than 24 hours with no date markings.  

La Quinta: 3380 W. Runkle Way 

One violation was due to the chemical sanitizer concentration being too high. According to the West Virginia University Sanitizing Fact Sheet, sanitizer should range from 50 to 100 PPM and should be a mixture of 1 tablespoon of bleach per gallon of water for sanitizing food contact surfaces.  

Lan Ramen and Aqua Tea: 250 S. Washington St.  

One violation was due to the hand sink needed to be clean at all times and used for no other purpose than hand washing.  

Lotus Garden: 110 S. Washington St.  

One violation was due to raw meat being stored above vegetables.  

McDonald’s #289 (North): 2300 N. Walnut St.  

One violation was due to debris buildup on the ice machine.  

MJ’s Kitchen: 275 E. Winslow Road  

One violation was due to breakfast sandwiches measured at 124°F maximum.  

Pizza X: 1791 E. 10th St.  

Two violations were due to: 

  • Chipped plastic containers being found.  
  • Food debris, dust and grease buildup on the pizza oven.  

Pizza X (Downtown): 312 E. Third St.  

Two violations were due to:  

  • Chemical sanitizer spray bottles reading at zero PPM.  
  • Mini-refrigeration unit kept at 56°F. According to the CDC, coolers should be kept at a temperature below 41°F. 

Planted Bloomington: Food Truck Friday 

One violation was due to a sanitizing spray bottle reading under 100 PPM concentration.  

Runcible Spoon: 412 E. Sixth St.  

Two violations were due to: 

  • Debris buildup in the ice machine.  
  • No date markings found on foods held over 24 hours.  

Rush Bowl Bloomington: 1412 N. Dunn St., Ste 3 

Two violations were due to:  

  • Buildup in the ice machine.  
  • Presence of a pest in the kitchen area by the mop sink.  

Smithville Food Mart: 7148 S. Fairfax Road 

One violation was due to no sanitizer being readily available.  

Soma Coffeehouse and Juice Bar (East): 1400 E. Third St.  

One violation was due to debris buildup on the ice machine.  

Speedway #1327: 3021 E. Third St.  

Two violations were due to: 

  • Food on rollers measured from 115°F to 123°F. 
  • Debris buildup was in the customer microwave.  

Speedway #6009: 3939 W. Third St.  

One violation was due to no air gap with hoses in the mop sink and three bay sinks.  

Starbucks Coffee #2670 (West): 248 N. Jacob Drive 

Two violations were due to: 

  • Air gap was missing from under the three-bay sink in the dishwashing area and barista area.  
  • Barista area sanitizer bucket reading zero PPM.  

Subway #10967: 1839 N. Kinser Pike 

One violation was due to cold holding of potentially hazardous foods not reaching 41°F.  

Viva Mas: 2550 E. Third St.  

One violation was due to scoops in bins for beans, flour and sugar. 

Vivencia Latin Flowers: 2800 E. Buick Cadillac Blvd.  

One violation due to debris in the ice machine in the coffee bar area.  

Wings Xtreme: 2620 E. 10th St.  

One violation was due to buildup in the pop nozzles and ice machine.  

Zero Degrees: 1285 S. College Mall Road  

Three violations were due to:  

  • Buildup in the ice machine and microwave. 
  • Raw chicken stored above cut leafy greens.  
  • Prepped food items being stored more than 24 hours without labels.
